Australia- MORE than 12,000 repeat warnings were given to the State Government about children allegedly at risk of neglect, abuse and sexual assault in one year.
And the Department of Human Services actually misplaced up to 2700 children in another year - having to alert police they could not locate those missing.

The truth is, you can't make a steak out of a turd, no matter how much A1 sauce you use.
When a government agency is operating in full compliance with the Communist Manifesto, there's GOT to be a problem or two with it.
